This plan is offered to all eligible employees in conjunction with AgileOne, a leader and strategic partner of ours in the staffing industry.

As an AgileOne supplier, you’re able to provide your eligible employees with Anthem Blue Cross, a trusted health insurance provider. This includes medical and ancillary benefits (dental, vision, and life).

Our insurance program guarantees that a client company can offer the same competitively priced minimum value medical insurance plan to its contingent workforce eligible for coverage, regardless of the various staffing companies supplying those employees.

PPO Dental Complete $1,000 Maximum benefit per person Insured. $50 Deductible per person insured. Preventive services covered at 100%. Basic services covered at 80% after deductible. Major services covered at 50% after deductible.

Dental DMO (CA Only) Subject to copays.

Vision Plan

Vision Plan

Blue View Vision $10 Exam Copay. Frame allowance $130, and then 20% of any remaining balance. Contacts are available in lieu of frames. Eye exams every 12 months. Replacement lenses every 12 months. Frames every 24 months.

Hospital Fixed Indemnity Plan

Hospital Fixed Indemnity Plan

Hospital indemnity insurance supplements your existing health insurance coverage by helping pay expenses for hospital stays. In general, most plans pay for:

Hospitalizations with or without surgery, Intensive care, and Critical care.

Voluntary Term Life Insurance You may purchase coverage in the amount of $20,000 (guaranteed issue). You may also choose additional coverage for your spouse and child(ren):
